A blameless postmortem is an incident retrospective run on the premise that the people involved acted reasonably given what they knew at the time, so the review hunts for systemic causes (missing guardrails, confusing tooling, bad defaults) rather than someone to blame. The idea traces to Sidney Dekker’s “Just Culture” work and was popularized in software by Google SRE and Etsy engineering. The mechanism is informational: punish the messenger and people stop reporting near-misses, so the organization goes blind to its own failure modes; remove the blame and honest reporting returns. This is the operational face of Westrum’s generative culture, and it feeds directly into the stability side of DORA Metrics, since teams that learn from incidents recover faster and repeat failures less.
